Chelmsford Public Schools has partnered with Remedium Pharmacy of North Chelmsford to offer Covid-19 vaccines to its students aged 12 and older Thursday, June 3, from 1:00 until 6:00 at Chelmsford High School.
Follow-up doses are scheduled for Thursday, June 24 from 1:00-6:00.
How to register your child
Visit the Remedium Registration Page to register and select a time for your child to receive their first dose of COVID vaccine on Thursday, June 3 (Select Remedium’s address — vaccinations will be administered at Chelmsford High School).
At the time of registration, please provide your insurance information for your child(ren) (If you don’t have insurance, you are still welcomed to participate in this vaccination program).
Download a copy of the consent form, sign it and have your child(ren) bring it with them on the day of their vaccination.
- Middle school students must be accompanied by an adult to receive their vaccine.
- Adult attendance is preferred though not mandatory for our high school students.
- Please arrive at CHS at your selected time.
